The written request from the receiving Vietnamese agency or organization;
List of foreigners directly participating in medical examination and treatment, including personal information, titles, expected jobs and practice licenses certified by the receiving agency;
List of interpreters for foreigners, including personal information, professional qualifications, and commitment to translation quality;
List of medical equipment (if any), including name, origin and condition of the equipment, and agreement on equipment supply if using domestic equipment;
Drug prescription (if any), including drug name, active ingredient, expiration date, and circulation number, ensuring the drug is legally circulating in at least one country;
Document of agreement between Vietnamese agency or organization and foreign person/agency on performing medical examination and treatment.
Note:
In the case of using medical equipment from a domestic medical examination and treatment facility, it is necessary to attach the equipment supply agreement of the agency where the implementation is expected.
If a foreigner intends to give or donate used medical equipment after completing medical examination and treatment, the receiving Vietnamese agency must carry out reception procedures according to Government regulations.
