Step 1: Submit application:

  • The applicant for re-issuance of the certificate shall submit the application to the Department of Health of the province or city where he or she practices by mail or in person.
  • After receiving the application, the Department of Health returns to the applicant for re-issuance of the Pharmacy Practice Certificate the application receipt according to Form No. 01 Appendix I of Decree No. 54/2017/ND-CP.

Step 2: Processing documents:

  • Within 5 working days from the date of receiving the dossier, the Department of Health will review and request supplements if there are any shortcomings. If the application is complete, the issuing agency will review and complete the certificate re-issuance procedure.
  • In case there is a request to amend or supplement the dossier, the Department of Health must send a written notification of the content of the request for amendment or supplement to the requester within 05 working days from the date recorded on the Form. receive documents. Within 06 months from the date the Department of Health issues a written notice of amendments and supplements, the applicant for a Pharmacy Practice Certificate must submit the application for amendments and supplements as requested. After the above deadline, if the applicant does not make any amendments or supplements or after 12 months from the date of first submission of the application, if the additional application does not meet the requirements, the submitted application is no longer valid.
  • After receiving the revised and supplemented dossier, the Department of Health receiving the dossier shall return to the applicant a Receipt of revised and supplemented dossier according to Form No. 01, Appendix I of Decree No. 54/2017/ND-CP. In case the revised and supplemented dossier does not meet the requirements, the Department of Health receiving the dossier shall issue a written notice to the applicant for the Certificate of Pharmacy Practice to proceed with the revision and supplementation.

Step 3: Reissue certificate:

  • The time to re-issue the certificate is within 5 working days from the date of receipt of complete and valid documents. If the grant is refused, the Department of Health will provide a document stating the reason.
  • Within 05 working days from the date of issuance of the Pharmacy Practice Certificate, the Department of Health shall publish and update the following information on the unit’s electronic portal:

Full name, date of birth of the person granted the Pharmacy Practice Certificate;

Pharmacy practice certificate number;

Scope of professional activities.
