1. For commercial vehicles. Profile includes:

• Application for reissuance of license according to Form No. 04 Appendix V of Decree 119/2021/ND-CP;
• A copy of the Vehicle Registration Certificate or a copy of the appointment letter to receive the Vehicle Registration Certificate from the registration authority or a copy from the original of the Vehicle Registration Certificate. In case the vehicle is not owned by the transport business unit, a copy of one of the following documents must be presented: A written vehicle rental contract with an organization or individual or a service contract between a member and a cooperative or a business cooperation contract;
