  1. Reissue due to loss or damage of practice license:
  • Application for reissue (according to form Form 08 Appendix I promulgated together with this Decree);
  • Original or copy of practice license (if any);
  • 02 portrait photos 4×6 cm, white background, taken within 06 months up to the time of application.
  1. Reissue due to changes or errors in license information:
  • Application for reissue (according to form Form 08 Appendix I promulgated together with this Decree);
  • Original copy of issued practice license;
  • Original or valid copy of documents proving changes or errors in information;
  • 02 portrait photos 4×6 cm, white background, taken within 06 months up to the time of application.
  1. Re-issued due to revoked practice license:
  • Necessary documents as prescribed in Article 14 of Decree 96/2023/ND-CP;
  • Original or valid copy of decision to revoke practice license (if any);
  • Other documents depend on the specific situation of the practitioner corresponding to the cases of license revocation.
  1. Re-issued because the practice license was issued without the right authority:
  • Application for re-issuance (according to form 08 Appendix I);
  • Original copy of professional license;
  • 02 portrait photos 4×6 cm, white background, taken within 06 months up to the time of application.
