  • General list of good health certified by the facility owner or certificate of health of the facility owner and people directly producing and trading food issued by a district-level medical facility or higher;
  • Certificate of training in food safety knowledge of the person directly producing and trading food, certified by the facility.

In case of request to re-issue the Certificate due to changing the name of the facility but not changing the owner, address, location and entire production process and business items:

  • Application for issuance according to Form No. 01b, Section I in the Appendix issued with Decree 17/2020/ND-CP;
  • Certificate of establishment meeting food safety conditions has been issued (copy certified by the establishment owner);
  • Documents proving the change in facility name.

In case of request for re-issuance of Certificate due to change of facility owner but not change of facility name, address, location and entire production process and business items:

  • Application for issuance according to Form No. 01b, Section I in the Appendix issued with Decree 17/2020/ND-CP;
  • Certificate of establishment meeting food safety conditions has been issued (copy certified by the establishment);
  • General list of good health certified by the facility owner or certificate of health of the facility owner and people directly producing and trading food issued by a district-level medical facility or higher (certified copy) received from the facility).”

Execution order

The process of reissuing the Food Hygiene and Safety Certificate is carried out according to the following main steps:

The order and procedures for granting the Certificate are the same as for the first issuance
